ABSTRACT

With the younger onset of diabetes, the reproductive dysfunction caused by diabetes has received widespread attention. Hyperglycemia and insulin resistance, the main features of diabetes, are high-risk factors for male reproductive dysfunction. Obesity, the common co-morbidity of diabetes, may aggravate the progression of reproductive dysfunction. Glucagon-like peptide-1(GLP-1) and its receptor agonists improve the overall health status by lowering blood glucose, reducing body weight, and inhibiting inflammatory response, which indirectly exerts protective effects on the reproductive system. GLP-1 also protects reproductive function by regulating the neuroendocrine function, and directly acting on the supporting cells and interstitial cells of the testis. However, some studies did not find the protective effects. High-quality clinical studies are needed. GLP-1 receptor agonists may be a therapeutic option to improve reproductive dysfunction in diabetic men.

ABSTRACT

The survival rate of cancer patients was improved due to the development of cancer treatment techniques, and thus the fertility protection for young female cancer patients has attracted increasing attention. Radiotherapy, as one of the comprehensive cancer treatment, could cause ovarian damage in adolescent and child-bearing women, which leads to fertility decline and a series of side effects. Radiation can cause ovarian damage not only by acting on biological macromolecules directly, but also by increasing oxidative stress between oocytes and ovarian granulosa cells indirectly. At present, the fertility preservation of female cancer patients undergoing radiotherapy mainly includes physical protection, drug protection and biological protection. Recently, the development of new technologies for the preservation of fertility in female cancer patients has also brought new hope, including factors such as protective effects, patient age, and the selection of specific cancer treatment measures, which are the main considerations in the selection process of fertility preservation measures. This article reviews the research progress on radiation-induced ovarian damage, with a focus on the introduction of the fertility preservation measures and new technologies for young female tumor patients receiving radiotherapy.

ABSTRACT

Objective@#To investigate the expression of phosphoglycerate mutase 1 (PGAM1) in the mouse testis after exposure to single heat stress (SHS).@*METHODS@#We randomly assigned 32 C57 male mice to an SHS (n = 16) and a control group (n = 16), the former bathed in water at 43 ℃ and the latter at 25 ℃ for 15 minutes. At 1 and 7 days after exposure, we harvested the testicular tissue for observation of the morphological changes of testicular cells by HE staining and determination of the location and expression of the PGAM1 protein by immunohistochemistry and Western blot.@*RESULTS@#The testis volume of the mice were reduced significantly, the spermatogenic tubules were disorganized, and the cells were reduced in number after heat stress and basically disappeared after 7 days. Immunohistochemistry showed extensive expression of the PGAM1 protein in the testicular spermatogenic tubules of the SHS-exposed mice, significantly higher than in the control group at 1 day after exposure, which was down-regulated in the testis tissue at 7 days, but still markedly higher than that in the control. Western blot exhibited significantly up-regulated expression of the PGAM1 protein after heat stress compared with that in the control group.@*CONCLUSIONS@#The expression of the PGAM1 protein undergoes dynamic changes in the mouse testis after exposed to single heat stress, which is related to heat stress-induced proliferation and division of testicular spermatogenic cells.

ABSTRACT

Background: The aim is to study various investigative modalities to diagnose Mullerian anomalies and to evaluate the reproductive outcome that occurs as a result of malformation of genital tract. Methods: A prospective study of reproductive anomalies and its outcome was performed by using data from women with congenital anomalies attended in OPD either with complains or for operative treatment or incidental diagnosis during caesarean section, manual removal of placenta, during laparoscopy or laparotomy at tertiary care center. Total 70 women with different types of congenital anomalies were included in study. Out of them gynecological cases (27) were diagnosed on basis of clinical examination and other diagnostic aids while Mullerian anomalies in obstetric cases (43) were observed and their outcomes were studied. Results: Most common utero-vaginal anomaly seen in present study was septate uterus with 18 (25.7%) cases, followed by bicornuate uterus with 13 (18.6%) cases. Most common presenting symptom is primary amenorrhea 21 (30%) followed by cyclical abdominal pain 11 (15.7). HSG, USG are the primary tools to detect genital tract anomalies. Surgical correction was required in 47.1% patients. Conclusions: Present study shows prevalence of congenital malformation of female reproductive tract is 0.17% at our tertiary care center. Utero-vaginal anomalies are a morphologically diverse group of developmental disorders. Establishing an accurate diagnosis is essential for planning treatment and management strategies. The surgical approach for correction of utero-vaginal anomalies is specific to the type of malformation and may vary in a specific group.

ABSTRACT

OBJECTIVE: To investigate the protective effect of cimetidine (CMTD) on rats irradiated with 60Co γ-rays at low dose rate.METHODS: Sixty male SD rats were randomly divided into normal group, model group, positive drug group, CMTD groups. The mice were irradiated with 60Co γ rays and an absorbed dose was 0.3 Gy, and the dose rate was 3.228 mGy•h-1. Twenty-four hours after irradiation, white blood cells, bone marrow DNA content, bone marrow polynuclear erythrocyte micronucleus rate (fMNPCE), sperm motility and sex hormones were detected. RESULTS When the cumulative absorbed dose was 0.3 Gy, the peripheral blood cells and bone marrow DNA content decreased significantly, fMNPCE increased significantly, and the sperm motility, follicle stimulating hormone (FSH), and luteinizing hormone (LH) levels reduced while the level of estradiol increased. Cimetidine not only could effectively increase the number of white blood cells, bone marrow DNA content, the level of FSH and LH but also reduce fMNPCE and the level of estradiol. CONCLUSION: CMTD could reduce the DNA damage which caused by 0.3 Gy cumulative irradiation, regulate sex hormone disturbance, and effectively improve hematopoietic function and reproductive function of irradiated rats. All RESULTS: shows that CMTD might be a good radioprotector.

ABSTRACT

Objective@#To investigate the role of pH2AX in the reversibility of mouse testicular reproductive function impaired by single heat stress.@*METHODS@#Twenty-four C57 male mice were randomly divided into heat stress and control groups and immersed in water at 43℃ and 25℃, respectively, for 15 minutes. At 1, 7, and 14 days of heat exposure, all the mice were sacrificed and their testis tissues collected for determining the apoptosis of the germ cells by TUNEL and measuring the expression level of the pH2AX protein by immunohistochemistry and Western blot.@*RESULTS@#The highest percentage of apoptotic cells were found in the seminiferous tubules of the mice in the heat stress group on the 1st day of the exposure and almost no apoptosis was observed at 7 and 14 days. The pH2AX protein was expressed in the nuclei of the basement membrane of adjacent seminiferous tubules. Compared with the control group, the expression of pH2AX was significantly increased on the 1st day of exposure (0.47 ± 0.02 vs 1.61 ± 0.04, P <0.01), then decreased at 7 days (0.85 ± 0.03) in comparison with that on the 1st day (P <0.01), and again elevated at 14 days (1.72 ± 0.02) as compared with either those at 1 and 7 days (P <0.01) or that of the control (P <0.01).@*CONCLUSIONS@#Heat stress causes dynamic changes of the pH2AX expression in the testis of the mouse, which are associated with heat stress-induced proliferation and division of the testicular spermatogenic cells.

ABSTRACT

Objective: To explore the effects of aqueous extract from Phthirusa pyrifolia leaves (67 mg/kg body weight for 12 days) on the reproductive function of male Wistar rats through oral administration. Methods: Animals (n = 30), aged 13 weeks and weighing (378.5 ± 5.0) g, were housed in a vivarium under controlled environmental conditions [photoperiod of 12 h light/dark, temperature of (23 ± 1) °C] and were fed standard rations ad libitum. The experiment ran for 12 days, wherein animals were divided into three groups: negative control (n = 6) received water, positive control (n = 12) with finasteride at a concentration of 1.0 mg/kg; and a test group (n = 12) submitted to aqueous extract. At the end of the experiment, the animals were sacrificed and submitted to analyses. Results: The morphological results of the testes showed that the aqueous extract induced significant changes in the diameter and cross-sectional area of the seminiferous tubules as well as the thickness of the seminiferous epithelium. Furthermore, the extract was able to abruptly decrease testosterone concentrations by about 81.88% in the treated group when compared with the negative control, (47.0 ± 4.8) ng/dL and (255.0 ± 2.0) ng/dL, respectively, and 76.8%, (211.0 ± 8.7) ng/dL, when compared with finasteride. However, the extract causes neither liver damage nor impairment of renal function. Conclusions: These results suggest that the high amounts of flavonoids shown to be in the extract may be responsible for its hepato-protective effects and suggest a possible decrease in the libido and reproduction of rats.

ABSTRACT

Zearalenone (ZEA) is a nonsteroidal estrogen-like mycotoxin widely distributed in maize, wheat, rice and other cereals with its derivants. It also presents in meat or dairy products or even in the aquatic ecosystem via rain, and thus can affect human health. ZEA affects the body function in various ways. On the one hand, it can disturb the synthesis of estrogen and its combination with the receptor, influence the reproductive ability via the estrogen signaling pathway, and cause the dysfunction of the reproductive systems. On the other hand, it can disturb the synthesis of DNA and proteins and result in lipid peroxidation and cytotoxicity by inducing the apoptosis of germ cells. It is known that exposure to different doses of ZEA can affect the female reproductive system by increasing the apoptosis of germ cells and inducing germ cell prematurity, sexual precocity, endocrine disorder, reproductive cycle disorder, and so on. But studies of its influence on the male reproductive system are relatively rare, especially about its unique male-related action mechanisms. This review presents an overview of the studies on the mechanisms of ZEA affecting male fertility and the phenotype changes in the male reproductive system after exposure to ZEA, hoping to provide some new ideas for the protection of human fertility.

ABSTRACT

Importation of rams of high genetic value is a common practice in Mexico; nevertheless, reproductive variables of imported animals have not been followed along. The objective of this study was to evaluate the reproductive function of Suffolk rams along their first reproductive season in Mexico (October to February). The behavior of values of scrotal circumference, sperm count and morphology, and circulating testosterone in Suffolk ram during its first reproductive season in Mexico are in line with the ones reported for Suffolk rams in other latitudes; however, in Mexico such changes are lower in magnitude. Neuroendocrine-reproductive changes induced by environmental stimuli such as changes in duration of light-dark cycles have been broadly documented and are accepted as inducers of seasonal changes in the ram's behavior. In this work, both testosterone concentration and sperm production were observed in December. These results show that Suffolk ram imported to Mexico maintain their cyclic testicular function, although it has a lower magnitude than in other latitudes. Causes of this condition are unknown, but they may be related to changes in the duration of the light-dark cycles prevailing in Mexico.

ABSTRACT

This paper examined the origins of the conception vessel (also known as the Ren meridian) based on references to the conception vessel in the Inner Canon of the Yellow Emperor (Huangdi Neijing, the seminal medical text of ancient China). It takes into consideration new findings related to medical principles, Chinese philology and graphology, and history. We reached the following opinions. It is clear from references in texts such as Lingshu Wuyin Wuwei's mention of underdeveloped innate reproductive functions in men, and the Bone Cavity thesis of the Suwen that states "diseases of the conception vessel appear inside the male as the "seven stomach aches"and in females as mucous discharges from the female genitals and lumps within the bowels", that the conception vessel has existed in both males and females since its discovery. The conception vessel runs along the body's midline, where the following are also located:the vestige of the umbilical cord connected to the fetus, the opening of the vagina and urethral opening (from which menstrual blood and semen are discharged, respectively), and the uterus which holds the fetus. Given that these are all located along the midline, it is thought that the conception vessel was formed based on knowledge of the meridian in ancient cosmology. Insight into the origin of the conception vessel can be provided by elucidating the mechanisms of how the human body gives birth to new life by placing focus on both male vitality and female menstruation while avoiding differences in genital structure. These are interpreted using the same criteria as kidney qi, heavenly dew, conception vessel, and the vital channel, suggesting that both share the same roots and are based in the bladder on the body's midline.

ABSTRACT

The incidence of small for gestational age (SGA) births is frequent, accounting for 2.3% to 8% of all live births. Several childhood and adult diseases are related to early postnatal growth and birth size, and 10% of children born SGA may have a short stature throughout postnatal life. Additionally, they may have abnormal growth hormone (GH)-insulin like growth factor axis, HPA axis, and gonadal function. Permanent changes are detrimental in an environment of nutritional abundance, and predispose SGA children to an array of diseases in adolescence and adulthood. Such changes may also cause premature pubarche, adrenarche, and precocious puberty. The varying results from clinical studies necessitate more prospective case control studies. Reproductive tract abnormalities and reproductive dysfunction are related to SGA births. GH treatment is required for SGA infants who do not experience catch-up growth.

ABSTRACT

OBJECTIVE: Treatment of ovarian tumor in pre- or perimenarcheal period dramatically improved, so the focus has been shifted to evaluated the reproductive and menstrual function. We checked pre- or perimenarcheal patients with ovarian tumor including malignancy to determine how the treatment affects patient's reproductive and menstrual function. METHODS: Clinical records of 74 pre- or perimenarcheal ovarian tumor managed at Ajou University Hospital from 1994 through 2004, were reviewed. All the patients underwent fertility preserving surgery and chemotherapy was added in some advanced malignant cases. Then we checked menstrual and reproductive status by history and physical exam, endocrine evaluation, pelvic ultrasonography, etc. RESULTS: In 74 evaluable patients, 46 cases (62.2%) were benign, 28 cases (37.8%) were malignant nature. The mean age of the premenarcheal patients at the time of diagnosis was 8.37 years (range, 3.2-14.0 years), and that of the perimenarcheal patients was 13.87 years (range, 11.5-16.9 years). The number of patients with benign tumor was 21 cases (75.0%), and that of malignant tumor was 7 cases (25.0%) in premenarcheal patients, and 25 cases (54.3%) and 21 cases (45.7%) in perimenarcheal patients respectively. The most common diagnosis in 46 cases of benign tumor was mature teratoma which constituted 82.6% (38 cases). Immature teratoma accounted for 8 cases (28.6%) in 14 cases of malignant germ cell tumor. In the patients treated with operation followed by chemotherapy 4 cases (50.0%) were suffered by menstrual problems and follow up data showed the ratio of menstrual problems was increased to 75%, which revealed statistical difference (p<0.05). Only one case (2.6%) in 38 cases of surgery only group experienced menstruation change such as period, duration, dysmenorrhea, and 3 cases (37.5%) in 8 of surgery and chemotherapy group were suffered from menstrual change (p=0.019). None of menstrual changes last permanently. CONCLUSION: After fertility sparing operation with/without adjuvant chemotherapy, majority can anticipate normal menstrual function. Anticancer treatment especially chemotherapy can affect ovarian function but it may be transient. This information is expected to prove helpful in the future counselling of such patients.

ABSTRACT

The present study was to investigate in more detail the changes of reproductive function in the male rat following myocardial infarction (MI). Ligation of the left coronary artery was performed in male Sprague-Dawley male rats at 60 days of age. Control rats were obtained sham-operated animals. MI rats were sacrificed at 1, 3, 5, 7, 14, and 30 day after ligation of left anterior descending coronary artery. Control rats were sacrificed on 30 day after thoracotomy. Myocardial infarct size was assessed by planimetry and perimetry. Testes of rats were fixed by whole body perfusion using a fixative containing 2.5% glutaraldehyde in cacodylate buffer, processed and embedded in Epon-araldite. Using 1 micro sections stained with methylene blue-azure II, qualitative and quantitative (stereological) morphological studies were performed. Testosterone levels in the incubation medium of luteinizing hormone-stimulated (100 ng/mL) testosterone secretion per testis in vitro, and in serum were determined by radioimmunoassay. Sperm production was measured by routine technique. Mean infarct size was 29.5~33.5% of the left ventricle after coronary occlusion in experimental groups. No changes were observed in testis volume, absolute volume of Leydig cell, Leydig cell size, and number of Leydig cell per testis in MI rats compared to sham-operated animals. Serum testosterone, LH-stimulated testicular testosterone production, and daily sperm production in MI rats were not significantly different (P>0.05) from sham-operated animals. These results demonstrate that under the experimental conditions employed here, experimental chronic myocardial infarction does not exert adverse effects on the reproductive function of male rats.

ABSTRACT

PURPOSE: As the survival rate of children with malignancies has increased over past decades, the follow-up in adult long-term survivors of childhood malignancies should focus on late effects of disease and treatment. This study was undertaken to find out whether sexual development was affected by the previous chemotherapy and reproductive function could be evaluated by Tanner stage and serum sex hormone level. METHODS: Pubertal stage and gonadal function were studied in 15 male adults survived 4.3~14.3 years after treatment for acute lymphoblastic leukemia, malignant lymphoma or lymphoma-leukemia during childhood or adolescence. RESULTS: All patients showed more than stage IV sexual maturity rating. Patients treated with cyclophosphamide including maintenance (CY group) had lesser testicular volume (P=.0001). All patients except one who has testicular involvement at diagnosis, showed normal follicle-stimulating hormone, leutenizing hormone, and testosterone level. Semen analysis was done in 2 patients. One patient with Non-CY group showed normal, whereas one with CY group showed azoospermia. It seemed that treatment period (before or during puberty) or prophylactic cranial radiation therapy did not affect sexual development.
